Basic Requirements getting the Services
While returning back after working on the ship, the sailors are required to submit the application to Shipping Division of Department of Marine Administration.
Requirements to submit the application:
- Original of Testimonial Form issued at the time of travel by DMA
- Application Form () to apply to DMA
- Copy of COC
- Copy of CDC
- Copy of Passport
- If services in the sea and D/R were obtained, the applicant needs to report with the copy of matriculation examination certificate in person before the end of one (1) month of returning back.

Required Documents
If the applicant is Crew, the following shall be submitted –
- the matriculation examination (original and copy),
- COP (original and copy),
- CDC (original and copy) and
- Passport (original and copy).
If the applicants are Officers, the following shall be submitted-
- COC (original and copy),
- CDC (original and copy) and
- Passport Copy.
If the applicants are graduates from MMU, MMMC, the following shall be submitted-
- Graduated Certificate
- MOA
- Training Record Book
- COP (original and copy),
- CDC (original and copy) and
- Passport (original and copy).
Applying Steps
- The applicant has to report to DMA office – Shipping Division (Training Section) in person with the stamp of local company and the signature of director on the testimonial form and on two (2) applications that are bought from DMA office.
- The applicant has to report to the respective officer of Shipping Division together with the necessary documents of sailor in person . If the submitted documents are perfect and completed, the respective officer will return the testimonial form to the sailor within one (1) week after reviewing.
